About The Job

Provides nursing services to patients and families at end of life that include assessment, planning, intervention and evaluation in the home setting or nursing home.

What We Are Looking For

Education [formal]:

Current IOWA RN license is required

Completion of physical assessment course preferred

BSN preferred

Hospice and Palliative Care Certification preferred

Experience

Must have demonstrated nursing competence in a health care setting for two years

Caring for terminally ill patients/families, Hospice or home care nursing experience is preferred

Special Skills And Competencies

Able to work autonomously yet work within a team relationship

Belief and knowledge of the hospice philosophy

Ability to prioritize, plan, organize, implement, and coordinate patient care services

Current motor vehicle driving license, motor vehicle insurance and availability to travel is required Availability to service patients in a variety of home settings including after business hours.

Obtain Port Access Certification within one year of hire date.

Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
